Key points
- Unahin ang PhilHealth: accredited hospitals deduct it from your bill automatically.
- Sign the CSF/CF1; the hospital and doctor complete CF2 and file the claim.
- No Balance Billing applies to eligible members in government hospital wards.
- Reimbursement must be filed within 60 days of discharge (180 days if abroad).

- Under the Universal Health Care (UHC) Law, all Filipinos are automatically PhilHealth members.
- Direct contributors: employees, self-employed, voluntary members, OFWs. They pay premiums.
- Indirect contributors: indigents (Listahanan/4Ps), senior citizens, PWDs and others whose premiums are paid by the government.
- Dependents of a member, such as a legal spouse, children under 21 and parents 60 and older who are not members themselves, can use the member’s benefits.
Magkano ang contribution (2026)?
According to PIA, the 2026 premium stays at 5% of monthly basic income, with a ₱10,000 floor and ₱100,000 ceiling. That means ₱500 to ₱5,000 a month. For employees, the premium is split equally with the employer. Self-employed and voluntary members pay their own share.
Tip: for direct contributors, make sure your contributions are updated, lalo na yung mga buwan bago ma-confine. Kung may kulang, ask PhilHealth or your employer agad.
Ihanda ang PhilHealth ID/PIN at MDR
- PhilHealth Identification Number (PIN): your 12-digit number. Ito ang hinihingi sa claim forms.
- Member Data Record (MDR): shows your info and declared dependents. Make sure tama ang pangalan, birthday at dependents.
- PhilHealth ID: helpful but your PIN plus a valid government ID is usually enough.
- You can get or check your PIN and MDR through the PhilHealth Member Portal, the eGovPH app, or any PhilHealth office (LHIO).
Ano ang sakop ng PhilHealth?
Inpatient (case rates)
Hospital confinement is paid through case rates, fixed amounts per illness or procedure that cover both hospital charges and doctor’s fees. PhilHealth raised many case rates starting 2025. Ask the billing office kung magkano ang case rate ng diagnosis mo, since amounts depend on the specific case.
No Balance Billing (NBB)
If you are an indigent, sponsored, senior citizen or PWD member admitted in a ward (basic accommodation) of a government hospital, No Balance Billing applies. Ibig sabihin, wala ka nang dapat bayaran beyond PhilHealth for covered services. Tell the admitting section agad na NBB-eligible ka.
YAKAP (formerly Konsulta)
YAKAP is PhilHealth’s outpatient primary care package: consultations, selected lab tests and medicines at your registered YAKAP clinic. Kailangang mag-register muna sa isang accredited YAKAP provider. PhilHealth pays the clinic directly, so walang claim forms na ifi-file ang member.
Z Benefits
For serious illnesses like certain cancers, kidney transplant and some heart surgeries, PhilHealth has Z Benefit packages. These are available only at contracted hospitals and require pre-authorization, kaya ask your doctor or the hospital’s PhilHealth CARES desk early.
- Pirmahan ang Claim Signature Form (CSF) at/o Claim Form 1 (CF1), which is the member/patient info form. The hospital staff will help you fill it out.
- The hospital and doctor accomplish Claim Form 2 (CF2) and the other clinical forms.
- Bago ma-discharge, check your Statement of Account (SOA) kung naibawas na ang PhilHealth.
- The hospital files the claim with PhilHealth.
- PhilHealth PIN and/or MDR
- Valid government ID of the patient and member
- Properly filled CF1 / CSF (signed by member or representative)
- For employed members: employer certification in CF1, if hinihingi
- For dependents: proof of relationship (PSA birth or marriage certificate) kung hindi pa naka-declare sa MDR
- For senior citizens/PWDs: senior or PWD ID
Reimbursement: kung hindi naibawas sa ospital
Kung na-confine ka abroad, or in limited cases like an emergency in a non-accredited facility, you may file for reimbursement directly at a PhilHealth office. Based on PhilHealth’s claim form reminders, file within 60 calendar days from discharge for local confinement and 180 days for confinement abroad. Bring CF1, CF2, the official receipts, SOA, and the other supporting documents the office asks for. For abroad, documents should be translated to English.
Tips para hindi ma-deny ang claim
- I-update ang MDR at dependents bago pa magkasakit.
- Huwag palampasin ang 60-day deadline.
- Kumpletuhin ang pirma sa forms. Incomplete forms can be returned or denied.
- Ask for the PhilHealth CARES desk or billing office kung may problema.
- Kung may balance pa after PhilHealth, apply for DSWD AICS, PCSO MAP, Malasakit Center or LGU assistance.
Questions people ask
Under UHC, all Filipinos are covered, pero for direct contributors, the hospital or PhilHealth may check your contributions.
Pero No Balance Billing applies only to eligible members in government hospital wards, so may babayaran ka pa sa private.
Paano kung wala akong PhilHealth ID?
Your PIN and a valid government ID are usually enough. You can get your PIN through the member portal or a PhilHealth office.
Pwede bang dependent ang magulang ko?
Yes, if they are 60 and above and not PhilHealth members themselves. Declare them sa MDR mo.
