Who the EAMC YAKAP Clinic is for

The clinic serves PhilHealth members and dependents whose registered YAKAP provider is EAMC. If your YAKAP clinic is a different facility (for example a health center or private clinic near you), you avail of YAKAP there instead, unless you change your provider first.

PhilHealth YAKAP (Yaman ng Kalusugan Program) is the newer version of what used to be PhilHealth Konsulta. It focuses on primary and preventive care so that people get checked before an illness becomes serious.

What is free under YAKAP at EAMC

  • Consultation
  • Laboratory tests covered by the program
  • Medicines covered by the program

Only the tests and medicines included in YAKAP are free. Anything outside the program list may still be charged, so ask the clinic staff if you are unsure about a specific test or drug.

Clinic schedule

  • Days: Monday to Friday
  • Registration: 7:00 AM to 2:00 PM
  • Referrals accepted: until 4:00 PM

Come early. Registration closes at 2:00 PM even though referrals are still received until 4:00 PM.

What to prepare before going to the OPD Triage

EAMC lists three requirements to complete before you get a number at the OPD Triage:

  1. YAKAP Provider Verification. Make sure your assigned YAKAP provider is EAMC, and present a valid and registered YAKAP Provider Verification.
  2. Consultation with your Primary Care Physician (PCP). You should already have consulted your PCP and received a referral under YAKAP.
  3. Laboratory request and prescription (reseta). Bring the lab request and prescription from your PCP for the tests and medicines covered by YAKAP.

Having all three ready makes the process faster at the triage desk.

How to register or choose your YAKAP clinic

You need a PhilHealth Identification Number (PIN) first. Based on PhilHealth and PIA guidance, you can then select a YAKAP clinic through any of these:

  • eGovPH app: open PhilHealth, go to the YAKAP tab and choose “Add YAKAP Clinic”, then pick an accredited clinic.
  • PhilHealth Member Portal: log in with your PIN, choose the option to select a YAKAP clinic, search by province or city, and confirm.
  • Walk-in: visit a PhilHealth Local Health Insurance Office or an accredited YAKAP clinic for in-person registration.

We could not confirm from official sources whether EAMC currently has open slots for new YAKAP registrants. Check the clinic list in the eGovPH app or ask EAMC directly before choosing it.

Common problems

  • Your provider is not EAMC. You cannot avail of YAKAP at EAMC until your provider is changed. PhilHealth says a transfer may require a biometric and liveness check and an empanelment slip.
  • No PCP referral. Consult your primary care physician first; EAMC asks for the referral before triage.
  • Late arrival. Registration ends at 2:00 PM.
  • Inactive membership. PIA advises checking your contribution status in the eGovPH app or PhilHealth portal before registering.

If you need help with costs that YAKAP does not cover, such as confinement or expensive medicines, you can look into other government medical assistance.
